Forms

Alternative Education Application - If you are interested in the alternative school, please turn in this completed application to your counselor.
Early Graduate Permission - For seniors who have met graduating requirements and chose to graduate at the end of first semster of their senior year.
P.E. Waiver (Marching Band) - For juniors/seniors who participate in marching band and chose to waive PE for the fall semester.
P.E. Waiver (Sports) - For juniors/seniors who participate in an IHSA sport and chose to waive PE while they are in season.
Permanent Study Hall Pass - For students who would like to be permanently reassigned to a particular teacher's classroom during their study hall. Teacher permission required.
Study Hall Waiver - For juniors/seniors who chose to wiave their 1st and/or 7th hour study hall.
Transcript Request Form - For current students who need an official transcript sent. Complete and turn in to your counselor.
